We welcome referrals from healthcare providers for patients who may benefit from specialized sexual health assessment or management. Our team consists of family physicians with a focused interest in sexual health medicine, offering comprehensive, evidence-based, and inclusive care to patients across Ontario.

To refer a patient, please complete our Referral Form and include all patient demographic information, as well as the referring provider’s billing number, phone number, and fax number.

Important Information:

• Referrals are accepted for patients with a valid Ontario health card (or other provincial coverage), IFH, or UHIP.

• Referrals are accepted for patients with no health card but the patient will need to pay for their appointment.

• Our office will contact both the referring provider and the patient to confirm and communicate appointment details.

• For physicians practicing within a FHO or FHT, please indicate the patient’s rostered status.

• Submitting a formal referral allows our physicians to bill a consultation code without affecting your own billing.

• Please note that in-basket procedure codes may result in negation; if you are referring a patient for gender affirming care, we suggest that you deroster the patient but keep them enrolled during the initial months of getting them started on hormone therapy.
